By registering for, participating in, viewing and/or listening to any web or audio conference sponsored by the Health Care Association of Michigan (HCAM) and/or the Michigan Center for Assisted Living (MCAL), I expressly accept and agree to the following terms and conditions of use:

Registration
Individuals who wish to participate in any live or on-demand web or audio conference sponsored by HCAM and/or MCAL must register for same via the designated online registration process. Individuals who have registered and paid, as applicable, to attend any live or on-demand web or audio conference sponsored by HCAM and/or MCAL may access and view, as applicable, both the audio and web components and print a copy of the instructional materials for his or her own use.
Permitted Uses
Other than these permitted uses, individuals registered to participate in any live or on-demand web or audio conference sponsored by HCAM and/or MCAL may not share, copy, distribute, display, reuse or resell any associated instructional materials. For example, participants may not:
- Download, copy or electronically transmit significant portions of the instructional materials (as determined by HCAM and/or MCAL);
- Download or save a copy of the audio and/or video files;
- Obtain instructional materials for anyone not registered for a virtual education program, with the exception of other staff members who work in the same building and do not wish to earn continuing education hours; or
- Share usernames, passwords, links or access codes with anyone not registered for a virtual education program.

These services and information do not constitute legal advice and their use is not a substitute for the independent, professional judgment of virtual education participants.
Copyrighted Materials
All live and on-demand web and audio conferences and instructional materials sponsored by HCAM and/or MCAL are copyrighted. All rights belong to HCAM, MCAL and/or its third-party contributors, as applicable.
Membership Status
During registration, individuals will be asked to select their membership status. Following are the three available options: HCAM Member, MCAL Member or Non-Member. HCAM and MCAL members include both facilities and vendors in good standing. Membership Number
HCAM and MCAL members will be asked to verify their membership status during registration by entering a membership number. HCAM members should enter their six-digit facility identification number and MCAL members should enter their National Center for Assisted Living (NCAL) membership identification number, which ranges from four to seven digits. Continuing Education
Each licensed/certified professional who wishes to earn approved continuing education hours for a live or on-demand web or audio conference sponsored by HCAM and/or MCAL must separately register and pay. This individual will only be eligible to earn approved continuing education hours if all valid Michigan license/certification numbers are submitted during registration. Following are the continuing education acronyms currently used by HCAM and MCAL:
- LNHA (Licensed Nursing Home Administrator)
- AFCL/A (Adult Foster Care Licensee or Administrator)
- RN/LPN (Registered Nurse/Licensed Practical Nurse)
- LSW/SST (Licensed Social Worker/Social Service Technician)
- CAP (Certified Activity Professional)
- CDM/CFPP (Certified Dietary Manager/Certified Food Protection Professional)
- RD/DTR (Registered Dietitian/Dietetic Technician, Registered)
Additional Participants
Registrants are encouraged to invite other staff members who work in the same building to participate—at no additional cost—in any live or on-demand web or audio conference sponsored by HCAM and/or MCAL for which he or she is registered and paid, as applicable. Additional participants who have not separately registered and paid, as applicable, are not eligible to earn continuing education hours.
Evaluations
At the end of every live or on-demand web or audio conference sponsored by HCAM and/or MCAL, registrants will receive an evaluation. Completion of this evaluation is required to earn a certificate of attendance. Individuals who do not participate in the entire web or audio conference—whether live or on-demand—will not receive an evaluation and will therefore not be eligible to earn continuing education hours for that virtual education program.
Certificates of Attendance
A certificate of attendance unique to each registrant will be available for print upon submission of a completed evaluation. Additionally, certificates of attendance will be e-mailed directly to conference registrants for printing and archiving purposes.
Confirmations
A confirmation will be sent via e-mail to each registrant. This confirmation will come from the e-mail address HCAM@CommPartners.com, so please add this e-mail address to your Safe Senders List. If you do not receive a confirmation within 48 hours of registering for a live or on-demand web or audio conference sponsored by HCAM and/or MCAL, send an e-mail to HCAM@CommPartners.com.
Method of Payment
All virtual education fees are due at the time of registration. HCAM and MCAL are pleased to accept Visa, MasterCard, American Express and Discover.
Security Code
The three-digit CVV2 number can be found on the back of all Visa, MasterCard and Discover credit cards. On American Express credit cards, the four-digit CVV2 number can be found on the front.
Cancellations/No Shows
Registered individuals who are subsequently unable to participate in a live web or audio conference sponsored by HCAM and/or MCAL may use the e-mail instructions provided for the live program to access an on-demand version of the same program beginning 48 hours following the live production. No refunds will be issued; however, the on-demand version may be viewed up to three times at no additional cost.
On-Demand
All on-demand (archived) web and audio conferences sponsored by HCAM and/or MCAL may be accessed and viewed, as applicable, within three months from the purchase date. Also, HCAM and MCAL reserve the right to remove any on-demand web or audio conferences from the On-Demand Virtual Education registration site when they no longer reflect current standards of practice.
Substitutions
Substitutions may be made at any time. Registered individuals who are subsequently unable to participate in a live web or audio conference sponsored by HCAM and/or MCAL and do not wish to access and view, as applicable, the on-demand version of the same program should send an e-mail message containing the name and e-mail address of the original registrant and the name and e-mail address of the substituting party to HCAM@CommPartners.com. The registrant data will be updated and an e-mail containing the virtual education access instructions will be sent to the new registrant.
Special Accommodations
HCAM and MCAL will make reasonable efforts to accommodate registrants who provide documented evidence of their disability or need for special accommodations with auxiliary aids and services that do not present an undue burden to the associations. If you require special accommodations, you must inform Aaron Wolowiec, director of member relations, of your needs at least two weeks prior to the scheduled start date of the virtual education program. Requests must be submitted in writing by e-mail to AaronWolowiec@hcam.org.
