Care transitions are a key area for performance improvement and measurement for many national leading quality organizations and have been identified as a priority in national initiatives.  When proper attention is not given to transitional care, re-hospitalization, emergency room use and medication errors go up. 

What are the recommended processes that the research indicates should be followed for optimum patient outcomes?  Where does the nurse’s role begin and end in the transition zone?

Maureen Dailey, DNSc, MSN, RN, WOCN  is the Senior Policy Fellow in the National Center for Nursing Quality at the American Nurses Association (ANA).  Dr. Dailey is responsible for nursing quality advocacy in the national leading quality organizations on behalf of ANA. 

 

She graduated from Columbia University with her Doctorate in Nursing Science and has experience conducting quantitative research investigating the relationships between the attributes of the work environment and nurse work outcomes.  Dr. Dailey has received various awards and honors of distinction and is a member of Sigma Theta Tau.

 

Her background includes patient and nurse safety, performance improvement, disease management and care coordination, education, telehealth, health policy advocacy; and program development, evaluation and management. She has published multiple articles in Home Health Care Management and Practice and other peer-reviewed journals. She co-authored the chapter “Creating a safe and high-quality health care environment in patient safety in quality”, in the 2008 Agency for Health Care Research and Quality (AHRQ) Publication: An evidence-based handbook for nurses.
